Understand what type of diabetes you have then talk to your doctor about how you can best care for your diabetes to stay healthy. Remember you are the most important member of your healthcare team.
Talk to your health care team about how to manage your A1C, blood pressure and cholesterol. This can help lower your chances of having a heart attack, stroke, or other diabetes problems.
You don’t have to be overwhelmed. Learn how to keep your stress level low, eat well, stay active (find an exercise routine that works for you), and take your medications. It’s important to stay connected to your health care team and let them know of any changes in your health.
See your health care team, at least, twice a year to find and treat any problems early. And if you have Medicare, check to see how your plan covers diabetes. It may include diabetes education, supplies, medicine and visits with a dietitian.
